Resources & forms for students
Students are required to submit specific forms to access various school benefits and privileges. These forms are essential for requesting permissions like leaving the premises for lunch, obtaining approval to drive and park on school grounds, and securing dedicated time for private study. To access and download these important forms, please find them listed below.
Lunch Pass
A Lunch Pass allows Esperance Senior High School students to leave the school premises during lunchtime. Students must only go home for lunch, not to the shops or other areas. This is a school requirement under Duty of Care.
Please fill in the required details, obtain signatures from the parent/guardian and student, and submit the below to the school administration for approval.
Class release
If you’re planning to participate in a camp, excursion, or any other school activity, you will need to inform and secure signatures from all relevant class teachers.
Once completed, please present the signed form to the designated activity coordinator for approval. Download the Class Release Form below.
Private study
Year 12 students who are on a pathway to graduation may have private study for the equivalent of one subject. A student may apply for a leave pass to do this study at home. It is expected that students use this time for Private Study.
Parents or guardians can download the required form below to apply for this private study leave.
Student vehicle form
If you plan to drive your own vehicle onto the grounds of Esperance Senior High School, you must apply for permission by completing the Student Vehicle Form.
Please download and submit the form after filling in the required details. Ensure that you read and understand the conditions stated on the back of the form before submitting your application.
